Medicaid's 'Cash and Counseling' Allows Pay For Family Caregivers

Mary, a 74-year-old Florida woman, suffered a stroke. She needed help with bathing, dressing, food shopping, meal preparation, laundry and housekeeping. Her state Medicaid plan covered the cost of a home-health care worker to provide those services, but the local agencies were short-staffed and couldn't send helpers on the schedule Anna needed.

So her daughter took time off work to care for Mary. Soon after, Mary and her daughter learned about Cash and Counseling, a non-traditional Medicaid program pioneered by New Jersey, Florida and Arkansas with seed grants from the U.S. Department of Health and Human Services, the Administration on Aging and The Robert Wood Johnson Foundation. 

Cash & Counseling participants may use their Medicaid-provided personal assistance budgets to hire their own personal care aides as well as purchase items or services, including home modifications that help them live independently. By redirecting personal assistance funds from agencies to consumers themselves, Cash & Counseling allows people to hire whomever they want to provide their care and decide for themselves if they would rather hire a home health aide to cook for them, or pay a friend or relative to do it. The services paid for by the state are all part of the elder's authorized Medicaid care plan. What's different is that, in many cases, family members and friends chosen by the elder are providing those services instead of an agency worker.

Medicaid's 'Cash and Counseling' Allows Pay For Family Caregivers

Today, the grants are available in 12 more states - Alabama, Illinois, Iowa, Kentucky, Michigan, Minnesota, New Mexico, Pennsylvania, Rhode Island, Vermont, Washington and West Virginia.

According to the Cash and Counseling website, the program was created because, "family caregivers are the backbone of the long-term care system, providing millions of hours of care every year for no compensation and frequently at great cost to their own emotional health. They're burned out and exhausted from juggling work, family responsibilities, and caregiving. In addition, many caregivers have to reduce their work hours or even give up their jobs to take care of their loved ones. By supporting caregivers, we're helping them hang in longer, and, hopefully, relieving some of their stress. Keep in mind, that even with Cash & Counseling support, family caregivers are typically paid lower-than-average wages and, in most cases, are paid for only a small fraction of the hours of service they provide."

To apply for the program, elders apply through Medicaid. They are assessed in the same way they would be for traditional agency-provided services. If they choose the Cash and Counseling option, they work with the program's staff to develop an individual budget and care plan.

An increasing number of children today are receiving a home school education. The reasons for making the choice to home school their kids varies from family to family but there are three main reasons why parents are removing their children from the public school system and giving them a home school education.

The first reason is that the public education system in the United States is struggling to provide a proper education for the nation's children with out of date text books, run down school buildings and inadequate equipment. Provision of a home school education enables the parents to have control over the quality of the educational materials used by their children and the general conditions in which they are educated.

The second reason is that parents wish to assume more control over the influences their children will be exposed to. Unfortunately, many public schools have a poor reputation for instilling good discipline in students. This often results in badly behaved children disrupting lessons and preventing their peers from getting the full benefit of classes. Discipline and the upholding of proper standards of behavior is an important part of a home school education.

The third reason many parents choose to give their children a home school education is fear for their safety. Violence is on the increase everywhere and the public school system has not escaped this trend. A home school education ensures the safety of children who would otherwise be seriously at risk of harm.

The Disadvantages of Opting For Homeschooling

Providing a home school education is not simply a matter of parental choice. In most cases the state education board of the state in which the family resides will have to approve a decision to give a child a home school education. The person taking on the responsibility of homeschooling must be certified to be a home teacher, the curriculum must follow the state curriculum, and the text books and other educational materials to be used must be approved by the state. Although this might seen like undue interference in what is a matter of personal choice, the state has a responsibility to ensure that all children receive an adequate standard of education and checks will be made to ensure that any child being kept away from public school is being properly educated.

A home school education might mean that a child is deprived of certain opportunities which would have been available within the public school system. There could be difficulties in providing facilities for athletic children to realize their potential. Musically talented children could be similarly disadvantaged. In some states there is provision for children receiving a home school education to take part in amenities such as being able to attend sports lessons and join after-school clubs. However, the level of assistance provided to homeschooling parents is not uniform and varies a lot from state to state.

The final potential disadvantage to affect children receiving a home school education is that they will not develop the social skills which will be important as they grow up. Social interaction with their peers and with adults outside the family is essential if a child is going to grow up with a properly balance personality and a reasonable level of social skills. These developmental issues can be fairly easily overcome if the child lives in a state where homeschooling parents are given support and the child receiving a home school education is accepted into classes and extra-curricular activities.

The decision to keep a child out of the public education system is not one any parent would make lightly and any weighing up of the pros and cons must take into account the level of support the state will provide. However, if the public school system continues to deteriorate, the number of children receiving a home school education is bound to increase.

The History Of Home Schooling

Home schooling is also known as home education, and is a method of teaching children in the family home, rather than at an institution, such as a public school. Originally, all schooling was done in the family home, or informally within small communities. Very few children ever went to school, or had private tutelage. Children who did have this type of education were considered to be privileged, and were mainly from wealthy families.

Informal education, mainly conducted in the home, was the only way for children to gain an education. In the US, there were books dedicated to home education, such as "Helps To Education in the Homes of Our Country" authored by Warren Burton. Parents were the main teachers of their children, although, where possible, local teachers would assist parents, and take classes. It is said that before schooling was institutionalized, the US was at its height of literacy skills.

The 19th century saw many significant changes to the way education, and schooling was conducted with the introduction of compulsory school attendance laws. It is now considered a human right that children are given an education provided by the government.

The History Of Home Schooling

Over the years, there has been much controversy over the effectiveness of institutionalized schooling, and some people have even gone as far as saying that the compulsory schooling system is damaging to younger children, especially boys who are slower to mature.

In the early 1970s, Ray and Dorothy Moore, who later become well known home schooling advocates, researched the bearing that early childhood education had on the mental, and, physical development of children between the ages of 8 to 12 years of age. Through these studies, the Moores produced evidence that formal schooling was damaging to children, and a cause for some behavioral problems commonly found in school aged children.

According to these tests, illiterate tribal mothers in Africa had children that were more socially, and emotionally advanced than children in the western world. The Moores believed that this was largely due to the bond between parents, and their children being broken when children were institutionalized in schooling systems.

In some English speaking countries, it is still an option for parents to home school their children rather than to send them to an institutionalized school. There are a wide variety of home schooling methods available to families who choose to home school their children, rather than send them to schools, including methods such as classical education, Waldorf education, and the Montessori method.

Home schooling can also refer to schooling done in a home environment, with supervision by teachers through correspondence schools. While children are schooled at home, they must still complete compulsory educational subjects, and take tests.

One of the main reasons that parents choose to home school their children is that they feel the schools are unable to offer their children the same quality of education, or social environment that can be taught at home.
